        Do you collect human subject data as part of your research study? The Safe Harbor method provides guidance on how to de-identify human subject data. Below is a quick guide to get you started on identifying details that require removal.

 

      NAMES
      

  • All proper names, even those not belonging to participants

      DATES
      

  • All elements of dates that are directly related to an individual

      GEOGRAPHIC LOCATIONS
      

  • Areas containing >20,000 people: only include first 3 digits of ZIP codes 
  • Areas containing <20,000 people: convert to 000 

      COMMUNICATION

  • Phone numbers
  • Fax numbers
  • Email addresses

      PHYSICAL

  • Biometric identifiers: finger and voice prints
  • Full-face and any comparable images

      INSTITUTIONAL

  • Social security numbers
  • Medical record numbers
  • Health plan beneficiary numbers
  • Account numbers

      REGISTRATION

  • Vehicle identifiers and serial numbers
  • Device identifiers and serial numbers
  • Web Universal Resource Locators (URLs)
  • MAC addresses
  • Internet Protocol (IP) addresses
  • Certificate/license numbers

      OTHER
